On the 23.12.2014 I, began my shift at 8am, on entering the building I signed in using the internal/external signing in book. Staff from the previous shift handed over all relevant information, which I then signed, I then checked to see what client I would be supporting on this particular day, today I was supporting client X, from the handover I had received information that client X had been verbally aggressive the previous day due to been agitated over Christmas and Christmas gifts, I then read Client X’s care plan and risk assessments to see of any changes- no changes had been made to Client X’s care plan.
At 10pm, I prompted to get client X up to comply with morning medications, Client X appeared in a sullen mood, Client X entered the medication room and prescribed medications were administered by myself and another staff member to witness sign to Client X. Client X’s then began to become verbal towards myself stating ‘I want to know what I’ve got for Christmas or I don’t want nothing at all’, I explained to Client X that Christmas was 2 days away and it wasn’t long now until she would receive her Christmas gifts, Client X then entered her room, myself and another staff member followed. Client X began to raise her voice stating ‘I want my Christmas presents now I want to go to my Mum’s house and get my Christmas presents’, at this point Client X then threw a deodorant can at the wall, I advised Client X against doing this and went on to explain to Client X that a home visit was arranged for Christmas day for Client X to go see her Mum. Client X continued to be verbal stating ‘I want to go today, I want my Christmas presents, I don’t like surprises, I want to know what I’ve got for Christmas’, I then again explained to client X that a home visit was arranged for Christmas day, I then spoke with Client X about seen her Mum on Christmas day and how much Client X had been looking forward to seen her Mum on Christmas day, Client X...
